Christian
T. Nielsen Unsung Hero Awards. STAR Awards will also present
the Christian T. Nielsen Unsung Hero Awards to honor both a volunteer
and employee in the health and human services field who go above
and beyond to serve people in need in Somerset County. An explanation
and criteria for this award and separate nomination forms are
attached.
We would like to offer you the opportunity to recognize your
volunteers for their accomplishments during 2004 by nominating
them for a STAR Award and/or a Christian T. Nielsen Award. By
doing so, you are making it known to the volunteers and the community
how important these special people are to you and your organization.
Feel free to share this website along with links to the online
forms and submit nominations for as many volunteers as you like.
Please note that one nomination form must be completed for each
nominee, with the exception of those being nominated as a group
or family. Nominations will be accepted from anyone in the community.

If you would like to make a nomination, you can fill out our
online nomination form using the nomination forms above until
March 10 or you can request a nomination form by contacting Melissa
Boronkas at (908) 253-6518 or mboronkas@uwscty.org.
Please return your nomination forms to Somerset County United
Way by March 10 (no extensions will be granted). In addition to
submitting online, forms can be mailed,
faxed (908) 725-5598, or e-mailed to mboronkas@uwscty.org.
Please include a picture of each volunteer to be included in our
STAR Awards video (photos will not be returned).
